Diabetes Services at the St. Anthony Chronic Care Center

Our diabetes team works with patients to make informed decisions every day about their diabetes care.

On a visit to our clinic, you will meet the diabetes team including a Registered Dietitian, Certified Diabetes Educator, and Advanced Nurse Practitioner. We offer a complete program in which you will learn everything you need to know about diabetes, pre-diabetes or gestational diabetes. You learn about using a meter, blood sugar testing, and medications. The dietitians will also help you develop a meal plan to fit your lifestyle. We also offer advanced therapies including continuous glucose monitors and insulin pumps. You are encouraged to bring a support person along such as your husband, wife, significant other, or a friend.

The staff at the Chronic Care Center is here to help you treat your diabetes, take charge, and manage it.
