On a visit to our clinic, you will meet the diabetes team. We offer a complete program in which you will learn what you need to know about diabetes. You learn about using a meter, blood sugar testing, and medications. The dietitians will also help you develop a meal plan to fit your lifestyle. You are encouraged to bring a support person along such as your husband, wife, significant other, or a friend.
The staff at the Chronic Care Center is here to help you treat your diabetes, take charge, and manage it.
The St. Anthony Chronic Care (Diabetes Services) Team is made up of:
- Registered Dietitians
- Certified Diabetes Educators
- Advanced Registered Nurse Practitioners
The Diabetes Prevention Program at St. Anthony Chronic Care Center has been
recognized by the Centers for Disease Control and Prevention at the highest level of clinical recognition. This designation is reserved for programs that have effectively delivered a quality, evidence-based program that meets all of their standards.
The Chronic Care Center's approach enables the delivery of real-time personalized coaching that achieves sustained outcomes. Diabetes educators work with dozens of the largest health plans, Medicaid, and self- insured employers to help their members prevent and manage chronic disease.
