Job Description

Job Summary

A Registered Nurse that coordinates the care and oversees MDS submission for skilled care patients.

Essential Duties and Responsibilities

• Determines appropriate patients for skilled nursing care in conjunction with the Nursing Director, Social Services, Physicians, and Quality Management, and Rehab staff.  

• Coordinate the care of the skilled care patients ensuring individualized updated appropriate care plans.

• Coordinate and direct a multidisciplinary skilled care conference weekly.

• Ensure appropriate MDS data collection, submission, and transmission.

• Evaluate, analyze, and maintains skilled care statistics on an ongoing basis, providing periodic reports to the Nursing Director and Quality Management Department.

• Analyze and interpret RUGS submission maximizing reimbursement.

• Uses Meditech Information System for nursing documentation as well as MDS submission.

• Provides quality nursing care through professional practice and clinical performance.

• Ensures that skilled care nursing practices are explained to patients and families.

• Ensures an appropriate education plan according to individualized patient needs.

• Consults with and keeps social worker and charge nurse informed of status and changes in skilled care patients.

• Continually demonstrates professionalism and works cooperatively with others.

• Participates in opportunities that enhance personal and professional growth seeking out continuing education focused on skilled nursing regulations, care, and/or reimbursement.

• Assists with Utilization Review processes and procedures as needed.

• Attends all mandatory meetings and programs.

• Performs other duties as necessary or assigned.

• Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.

• Complies with all applicable state and federal regulations.

• Maintains a safe work environment using safe work practices, utilizing standard precautions and complying 

  with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.

• Maintains the confidentiality of patient, resident, employee and organizational information.

Education and/or Experience

• Registered Nurse.

• Bachelor’s degree strongly encouraged.

Position Requirements, Licenses, Certifications, Registrations

• Graduate of accredited School of Nursing.

• Currently licensed to practice professional nursing in the State of Iowa.

• BCLS certification required upon hire.

• Prior skilled care knowledge preferred.

• Knowledge of MDS data analysis and submission, certification required within 1 year of hire.

Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write routine reports and correspondence.  Ability to speak effectively before customers, patients, families and employees of the organization.
