Job Description

SUMMARY: Provides outreach education, coordination of care, public relations to providers, child-bearing families, and Public Health in Carroll County and the six surrounding counties of St. Anthony Regional Hospital.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Meets with providers and their staff in the six county area surrounding St. Anthony Regional Hospital to strategize best care practices to meet the needs of child bearing families in their service region.

Meets with Public Health Clinics in the six county area surrounding St. Anthony Regional Hospital to strategize best care practices to meet the needs of child bearing families in their service region.

Provides education to clinic staff about services available at St Anthony Regional Hospital to families that are anticipating childbirth.

Meets with providers to strategize patient care coordination for families that are delivering from the outlying regions.

Develops a multi-disciplinary care plan with the families delivering at St. Anthony Regional Hospital from outlying providers that includes prenatal care and testing, childbirth education and follow-up care after delivery.

Possess knowledge of current patient care practices in The Birthplace.

Develops a plan for return of care to local providers in coordination with the delivering physician, family and local provider.

Communicates with The Birth Place staff as a member of the multi-disciplinary care team for families from the six county area.

Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.

Complies with all applicable state and federal regulations.

Attends all mandatory meetings and programs.

Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.

Maintains the confidentiality of patient, resident, employee and organizational information.

Travel amongst the six surrounding counties will be required at a minimum.

Completes all other duties as assigned.

This job has no supervisory responsibilities.

QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE

Associate's degree or equivalent from two-year college or technical school or diploma from a school of nursing.

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before customers, patients, families and employees of the organization.

POSITION REQUIREMENTS, LICENSES, REGISTRATIONS, CERTIFICATIONS

Graduate of an accredited school for professional nursing.

Current license to practice professional nursing in the State of Iowa.

Current BCLS certification.

OB experience preferred.
