Job Description

SUMMARY Under the supervision of the Director and professional nursing staff, functions as a member of the healthcare team by providing services and support for clients to attain/maintain independence and strengthen home life.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Provides quality and safe delivery of services to clients in their homes as assigned and supervised by RN staff according to the developed plan of care for each client of any age.

Assists/provides clients with personal care; activities of daily living, nutrition through meal preparation, teaching and emotional support.

Assists/provides environmental maintenance and management which are essential to the client’s health care at home and necessary to prevent/postpone institutionalization.

Reports any client changes, concerns and incidents promptly to RN.

Completes documentation, visit forms, and time studies within time limits specified.

Adheres to assigned schedule and notifies supervisor if unable to complete.

Acts as an advocate for the client.

Maintains a well-groomed appearance, conforms with Agency dress code and wears name pin on duty.

Assists clients to access specific preventive services as directed by professional.

Informs individuals of services available and their benefits.

Assists individuals in filling out or updating forms.

Attends coordination of care meetings.

Completes all other duties as assigned.

Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.

Complies with all applicable state and federal regulations.

Attends all mandatory meetings and programs.

Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.

Maintains the confidentiality of patient, resident, employee and organizational information.

SUPERVISORY RESPONSIBILITIES

This job has no supervisory responsibilities.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to communicate effectively with customers, patients, families and employees of the organization.

POSITION REQUIREMENTS, LICENSES, CERTIFICATIONS, REGISTRATIONS

Current CNA certification in good standing preferred.

Current BCLS certification.

Must have reliable transportation, current, valid driver’s license and be able to show proof of adequate auto insurance coverage.

Must be able to enter and function in non-handicapped accessible client homes.

Must be able to manage time and work independently.

The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 21 pounds, and occasionally lift and/or move up to 35 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, color vision, depth perception, and ability to adjust focus.
